The only feedback I have is maybe to reduce the History section, it has a lot of subsections. Maybe put something like "Suggestions for History: childhood events, adolescence, etc." instead of a subsection for each. The only reason I say that is from personal experience. I do like to conceptualize a lot about my character but there are some things I choose to develop and discover as I go, and if I had to fill out such an extensive application I might be turned off creating a new character.
The same with Personality. Instead of "General Personality" AND the Negative/Positive traits maybe do one or the other?
However, I would agree with keeping so many sections for Professors. We want people to put in effort and get attached to those characters so they stick around. We want there to be more investment in positions of power.
I do like that you've included Place of Birth and Former Education, I think those are often overlooked as important details. Prominent Features is also nifty Good job Willow!

It looks good though :3 Though I shudder to think what we'll see in the companion/pet section from some -.- but that's why we have apps! So we can see that and go FUCK NO.
Whenever I see profile applications, I always do a quick sanity check. Like, are there questions like "Favorite toothpaste flavor". XP I like that while reading over the questions in these apps I can think of an answer for each of them in less than a second. Still, I've been playing my characters for years now, so it's important to consider what it might be like for a making brand new character for a site that requires apps. Keep in mind that this isn't a profile we fill out for fun, with a lot of little details to think about. This is an application. For at least the student app, covering the very minimal basics to make sure they'll play nice is enough. This is their first hurdle entering the site, so we want to make this as simple and easy as possible.
I agree that just General Personality is enough for description. Possibly a likes/dislikes section. We don't need their entire psyche laid out for us, or police a perfectly balanced character, in my opinion. There's plenty of time to explore that later in RP. Just enough that we know there was thought put into it.
As for appearance, I've never put much stock into it. How much does it come up in RP, really? Hair/eye color and ethnicity is enough, plus other general distinguishing characteristics. Strike the body type and wardrobe. If it's important, they'll mention it on their own.
House, Wand, and Companion are great, basic things that every student should have. Thinking up a wand might be tricky, so I suggest directing them to a ready made quiz somewhere as well. This one is good. I also really like former house (assuming they're magical) and occupation for adults.
For History, strike it all and put them as suggestions instead. We don't want to force them to write a life biography. They can if they want, but if they don't, we don't want to make them go through extra effort. It should be just enough to catch any big issues.
For miscellaneous, I think Family (or lack of) is fairly important. I'd put it in another section. As for the rest of the relationships, if they're just entering, it's not likely they'll have friends or rivals or enemies unless they're entering with multiple characters or with real life friends. Dunno if that section's needed at all.
Strike Hobbies, Allergies, Fears, Tattoos / Scarification, Scars, and Piercings. They're all redundant and can go under previous categories. Just add a trivia section instead.
Now, all this is assuming that these are new student/adult applicants. Otherwise, I agree with Roan about having separate, more detailed profile applications for positions. I think the profiles as they are now are good for that, especially the Place of Birth and Prior Education for Professors. We want something solid for site leaders.

I like these. They're thorough without being too involved. I agree, though, with Anil and Roan. The Personality, Appearance, and Miscellaneous sections would do better freeform, with those sub-sections as suggestions. I also support the idea of a Family section, since (and maybe this is just me), but we seem to have quite a few brothers/sisters/cousins/other relations going on, and it would be interesting, if not just downright helpful to see who is related to whom.

For once I agree with a Hawkins.
Personally it turns me way off when I have to think through a character like, 100% through when joining a site. I especially don't like to have to describe appearance... but that's kind of one of my things. I think I spend a good few months (or more!) not having any idea of what my character actually looks like, and I like it that way. Same with clothing style/attitude, because I decided that on a thread-by-thread basis, but hey.
'positive' and 'negative' traits result in pigeon-holing your character into a certain 'type', or worse, making up random shit so your character is balanced. Positive and negative are relative to your own beliefs anyway. I think for students and generic wizards it would be better to have more general application requirements. Name, age, brief personality, brief history, etc.
I agree that for professors it should be a bit more in-depth.
To be honest I'm against applications to join a site but I think that it's a necessary thing in today's world of RP.
Good job though Willow. :3 Also bring Elle back for me to lez out with.
More suggestions? If so I'd say with the family/relationships to make that either optional or "If significant" or something. So like it'd be important for Anil/Ilmar Hawkins cause they're brothers. Or people who are cousins or who are related in some way. But my character Niobe, her parents and siblings don't really mean squat. I'm conceptualizing Margaret right now and I have yet to decide if she has siblings...
Plus isn't the friend/interest/rivals part a bit pre-emptive?

Random thing: I think I might like the teacher application to be a mini thing all by itself. Like, basically only like: "what job are you applying for, give a sample class" sorta thing. Thus, appropriately aged characters can get jobs in the school other than just adult humans.
